Check Insurance Coverage

If we do not accept your insurance plan, we will submit an insurance claim as a courtesy.  You must sign a financial agreement indicating that you are responsible for any fees that are not paid by your insurance.  You should contact your insurance company to confirm your coverage and benefits. Some plans will not pay benefits if you choose to see an "out-of-network" physician. Below is a list of participating insurances but it is recommended that you call our office before you visit at 518-489-4471 to confirm participation as this can change without notice.

Referrals

The Center for Rheumatology does require new patients to have a referral from their primary care doctor or another physician's office. In the case of the patients’ insurance carrier requiring an insurance referral, then it is the patient’s responsibility to obtain the referral from their primary care physician.

Complete Registration Paperwork
