We are inviting patients to call again for in office appointments. Here are the questions that we will ask you prior to scheduling an in office appointment.
Please be advised we are screening all patients for the following:
Have you had any of the following symptoms within the last 2 weeks: new onset of shortness of breath or cough, fever? Do you have any cold/flu like symptoms?
Have you traveled outside of NYS in the last 2 weeks?
Have you been in a nursing home, assisted living or rehab facility with in the last 2 weeks?
Have you tested positive for COVID 19?
Have you been in contact with anyone that has tested positive for COVID 19?
We are continuing to do Telehealth visits.
Your health care is our top priority.
